Established Patients:
Accessing Your Patient Portal:
As an established patient you can access your electronic medical records with ease. All you need in order to login to your secure account is your Username & your password.
Start by clicking the red "Patient Portal" button above. This will redirect you to the portal website. From here you want to login. If you have forgotten your username, just remember that we keep it simple with it just being your first & last names without any spaces (don't forget to capitalize!). i.e.: Jane Smith's username would be "JaneSmith"
If you have forgotten your password then you can choose the "retrieve login credentials" option below the username box. This will send a link to your email account that's on file.
Have you tried logging in with the incorrect credentials 5 times and are now locked out? Don't worry, just give the office a quick call and we can reset your account and password for you.
What is the Patient Portal?
Once you are logged in to your patient portal then you can view your (limited) medical records, view and change your current medication list, view recent referrals. (Note: these are not insurance referrals but interoffice referrals.) You can also see your reviewed lab results. If you have a non-emergency question then feel free to contact the office via portal messages. Need a routine refill? You can send an electronic message to us about that too! Keep in mind that this is NOT for urgent refills, for those you will need to call the office directly.
