We are happy to release your health record to any physician or clinic at your request. Please allow 3-5 days for records to be sent. Most offices do not want you entire record and only request the last 1-2 years. We do not want to overwhelm your new doctor so we only send what they request.

 

If you would like a copy of your entire record we are happy to copy your complete health record. Please provide a thumb/USB/flash drive of at least 4GB (per patient) to copy your records onto. Please allow 2-3 weeks to receive your entire record as we will be handling this in-house and are limited to only 1 person copying the records over.
