New Patient Questionnaire:
At least 1 business day prior to your New Patient Appointment
you MUST have your New Patient Questionnaires 100% complete.
If you do not have them complete then we will have
to reschedule your appointment and you will be
responsible for the $50 late cancellation fee.
In order to access the Questionnaires you will need to have a login and password for the Patient Portal. If you do not have these then you must contact the office to be assigned one. Once you are in the system you can access the Portal directly from our website, just look for the big Patient Portal button at the top of every page.

How To...

Use your login and assigned password to access your portal; you will be asked to verify your date of birth and one of your telephone numbers. You will also be required to create a security question.
You will then create your own password. Please ensure that your password remains private as it provides access to your medical health record.

Any messages from the Doctor or Office Staff will show up here. These messages will always be non-emergency related.
Your lab results will populate here ONLY after the doctor has reviewed them. Sometimes they will include comments from the doctor. If you still don't have any results or messages re your labs after 1 week then please contact the office.

Under "Questionnaires" select "Medical History" to open the questionnaires.
Each tab is a separate questionnaire. At the bottom of each questionnaire is a "Submit Request" button, you MUST submit before moving on to the next tab/questionnaire. If you do not submit then all of your data will be erased. Do NOT hit the submit button multiple times as that will also erase your information.
Once you have completed all of the Questionnaires in their entirety, you can always call the office to verify that there were no errors or incomplete questionnaires.
New Patients - What to Bring to your First Visit
We ask all of our new patients to provide us with whatever medical history or medical records you might have in your possession. If you do not have any actual records then please provide us with:

the names
-
specialties
-
phone numbers
- of any providers from the last 5 years or from any major medical procedures.
(Available printable physician list for your convenience.)
Bring along a Photo ID.
Active insurance card(s). You MUST have these with you in order for you to be seen by the doctor.
For your initial office visit we have you bring ANY & ALL medications that you are taking on a daily or 'as needed' basis. Please bring the medications in their original bottles or boxes to prevent any errors or later confusion. ALL medications include prescriptions, over-the-counter meds as well as any supplements that you are taking.
